Checklist for Older Adults
A room-by-room “Home Fall-Risk Map”
Most fall injuries among older adults happen not outdoors but in the home they know best. This check walks you through seven areas — from the entrance to the stairs — using simple room cards, so you and your family can look for places worth reviewing in their home.
- This is not a diagnostic or fall-prediction tool. It does not show fall probability or risk levels.
- There is no need to deliberately try risky movements. Recall everyday situations and, ideally, check the actual rooms together with your parent.
- Your answers stay on this screen only; nothing is saved or sent anywhere. Reloading the page clears them.
For each area, choose whether anything concerns you in everyday use. Selecting "Concerns me" shows the points to check.
"Not checked yet" marks places you have not yet looked at. Choose "Not applicable" for areas the home does not have (for example, no stairs).
No areas have been checked yet. Please start with the room cards above.

Also review changes in your parent's daily life
Alongside the home environment, our Parent Life-Change Awareness Check helps you organize changes in walking, meals, and outings from a family member's point of view — and suggests where to seek advice.

Frequently Asked Questions
We also welcome inquiries from family members
It is fine to reach out even at the "we don't know where to start" stage. At ZEN PHYSIO STRETCH, a licensed physical therapist assesses your parent's physical condition, and we welcome inquiries from family members. If a medical institution or a local public desk seems like the right first step, we will tell you so honestly.
